 BEFORE YOU DIVE

Dive Courses require three signed forms: A Liabiltiy Release/Assumption of Risk, a Safe Diving Practices Statement of Understanding, and a Medical Statement form. The forms are different forms depending upon whether one takes a PADI or SSI course. However, the medical form is the same for both.

Downloadable forms to print and complete (PDF documents):

Young people  less than 18 years old will need permission forms signed by a parent or guardian to take scuba diving courses.

E-mail us with any questions:  MissionImmersion@live.com

Are you fit to dive?

If you are a certified diver and it has been awhile since you have been diving, you might want to review the Medical Form with your physician to feel confident that you are ready to dive.

There are now internationally agreed upon recommendations for people with diabetes who dive. See a summary of those diving recommendations here.
